1. Work Health and Safety Policies and Procedures

For this part of the assessment, you will be required to familiarise yourself with the Vocational Placement Organisation’s policies and procedures on work health and safety.

2. Conducting Work Health and Safety Review

For this part of the assessment, you will be required to conduct a work health and safety review in your organisation.

This part is divided into two (2) tasks:

Part 1 – Safety Inspection

This part requires you to conduct a safety inspection of your vocational workplace. The objective is to identify hazards, assess associated risks, and develop control measures to risks identified.

  • Consultation, cooperation and communication
  • Evaluation and maintenance of WHS

1. Researching on Society and Diversity

For this part of the assessment, you will be required to conduct research about society and diversity to help you gain a better understanding of the context of service delivery in Australia and in your area.

 

Researching on Workplace Diversity

In this part of your research, you will:

  • Collect and evaluate quantitative and qualitative workplace diversity data
  • Identify and analyse potential benefits of diversity in relation to the workplace objectives

  • Monitoring and reviewing the effectiveness of work practices and services in meeting client needs
  • Modifying work practices and services for improvement to better meet client needs

To complete this assessment, you will need access to the following:

Steps to take:

  1. Consult with your supervisor/relevant personnel to assist you in scheduling a discussion with each of the three (3)

 

  1. In your meeting with the client, ensure to clarify with them your role and the purpose of your meeting. Inform them that the data gathered relating to them is for assessment purpose Ensure them that you will maintain the privacy and confidentiality of their information.

For this part of the assessment, you will require access to:

  • The organisation’s policies and procedures relating to fostering and promoting diversity
  • At least two (2) organisation stakeholders

 

Guidance: These must include people who will be affected by the changes in the policies and procedures such as your supervisor, colleagues, government workers, etc.

  • At least four (4) people, with two (2) persons each, from two (2) different key target groups

 

Guidance: These can include clients, their family members, or community members. There must be two (2) different key target groups with whom the organisation is working (e.g. people with a disability, homeless people, youth, Aboriginal and Torres Strait Islanders, etc.)

 

Prior to commencing this part of the assessment, ensure that you have discussed the resources required for this assessment, and have secured the necessary permissions for you to undertake this assessment within your organisation.

 

Steps to take:

  1. Obtain a copy of the organisation’s policies and procedures relating to diversity practices. Review the strategies that the organisation has in place to foster and promote diversity when working with colleagues, clients, and other

Task 2 – Consultation with Stakeholders and Key Target Groups

For this assessment, you need to schedule a discussion with the each of the following:

  • At least two (2) organisational stakeholders
  • At least four (4) people from two different key target groups of the organisation

 

Steps to take:

1. Consult with your supervisor/relevant personnel to assist you in scheduling a discussion with the stakeholders and clients.

 

2. Ensure to clarify with the stakeholders and clients your role, and the purpose of your meeting. Ensure them that you will maintain the privacy and confidentiality of the information gathered from the

 

3. In your meeting, you are to develop three (3) strategies* which can be implemented in the organisation, in line with the diversity policies and procedures reviewed. The strategies you need to develop must be able to achieve the outcomes of the organisation’s policy, as specified in the Diversity Policy and Procedures

To complete the assessment activities included in this part of the Skills Workbook, you will need access to:

You will need a software or mobile application for managing work priorities and tasks. (E.g. a calendar application, to-do lists, task scheduler, organiser, or other similar technology).

 

Steps to take:

  1. Plan and plot two (2) weeks’ worth of Vocational Placement work tasks and priorities using the software/mobile application you have chosen to complete this assessment with.

 The tasks and priorities you plot here must be relevant to your Vocational Placement.

  1. This Reflective Journal is made up of eight (8) journal entries.
  2. Each entry contains Reflection Guides. These guides will assist you in providing the information required to complete each journal entry.
  3. The Reflection Guides also contain instructions and activities you need to complete to enable you to provide the information required. A summary of requirements is outlined in the Reflective Journal Overview located in the following.
  4. When completing each journal entry, always provide complete responses, where required.
  5. Your responses must always be based on your experience during your Vocational Placement.
  6. Whenever the Reflection Guides tell you to do so, discuss your responses with your In some entries, the Reflection Guides will ask you to provide summaries of your discussions with your supervisor and other relevant stakeholders such as clients and co-workers.

3. Observation Form

This part of the assessment outlines all the tasks, skills and performance requirements relevant to undertaking personal and professional development.

You are required to complete the tasks outlined in the observation form provided for this subject according to the quality standards set by your vocational workplace, and in compliance with the industry standards relevant to the role of case workers.

Subject Overview

This part of the Skills Workbook requires you to contribute to the case management process and coordinating complex cases within your organisation.

 

This subject is divided into five (5) tasks:

Resources Needed for Assessment

To complete the assessment activities included in this part of the Skills Workbook, you will need access to:

  • A Vocational Placement Provider that will allow access to:

You may use the case management forms and templates available in your Vocational Placement Workplace. However, in case these are not available, you may use the ones provided within this Skills Workbook.

 

1. Intake Interview

For this part of the assessment, you are required to undertake an intake interview, on two (2) separate occasions, with two (2) clients. Arrange with your supervisor, access to clients with complex care requirements.

 

Steps to take:

1. Review your organisation’s policies and procedures in completing an intake interview for clients.

Guidance: These may have already been provided to you on your first day of Vocational Placement or during your induction and orientation in the service.

 

2. Request for your supervisor’s authorisation to access the referral form or the details of the client. Review the client’s details before proceeding with the intake interview.

 

3. Follow your organisation’s policy and procedure in completing the intake Ensure that you have:

(1) Introduced yourself and the service, and sought introduction from the

(2) Engaged the client by establishing rapport and trust.

(3) Identified the purpose of the interview, oriented the client, and sought the client’s

(4)Explained the following and sought the client’s feedback:

    • your role as case manager and case coordinator
    • the assessment process and its practical aspects
    • client’s rights and responsibilities, including rights to appeal and avenues for complaints
    • the methods of communication between you/service and the client
    • the procedures for release of personal information to another party and the requirement for informed consent for release

(5) Explored the most urgent issues the client is experiencing and prioritised these with the

    4. Encouraged the client to ask questions and raise concerns, if there are any

 

3. Conduct the formal assessment interview using the process employed by your Information collected from the client must at least cover the following:

Guidance: Ensure privacy and confidentiality all throughout the interview.

  • Presenting concerns
  • Past medical history and background
  • Previous agency/service involvements
  • Psychosocial and behavioural health
  • Psychosocial support systems

4. Summarise the information collected from the formal assessment interview and document it using the assessment form used by your organisation or use the template provided below.

 

5. Follow these guidelines in documenting the assessment interview:

    • Bullet points may be used in lieu of a narrative/essay
    • Paraphrasing may be used to convey the general idea of what is being
    • Record the client’s exact words using quotations marks if there is a need to highlight his/her statement(s).
    • Use simple but appropriate Slang may only be used if the client’s exact words are being recorded.
    • Write objectively, concisely in the active voice, and include only necessary

 

3. Develop Case Management Plan

For this task, you are required to develop an appropriate case management plan for each of your two (2) clients.

 

Steps to take:

  1. Before creating the case management plan, reflect on the outcomes of the intake and assessment interview to be able to identify and analyse the multiple and complex needs that you need to consider in developing the case management plan.
  2. Evaluate the needs of the client based on the information gathered during intake and Prioritise the client’s needs based on his/her responses from the previous interviews.
  3. Once the needs are identified, research and compile information on the availability and capacity of other services that can support the client. Keep in mind that a specialist’s input is of particular importance to support multiple and complex
  4. Develop the case management plan using the template provided below:

(1) List all known persons currently contributing to the client’s care. This includes the individual, carer/advocate, key worker/ case manager/ coordinator, specialists (e.g., GP, health/community providers, substitute decision maker, family members, volunteers or friends who provide assistance). The following must also be in the case management plan:

    • Full name of the participants involved in care
    • His/her role or kind of support he/she is giving the client
    • Contact details (e.g., phone, emails, )
    • Will the participant involved in care be included in the planning process?
    • Will the participant involved in care be provided with a copy of the care plan?

 

(2) Provide a summary of the outcome of the assessment interview in bullet points. This must include the identified multiple and complex needs of the client that the plan is meaning to address.

 

(3) Complete the Support Plan:

    • Under Opportunities, provide five (5) needs/concerns that were identified during the assessment List these needs/concerns in order of priority.
    • For each Opportunity, set two (2) goals that are SMART— specific, measurable, attainable, realistic, and time-bound.
    • For each goal, identify the action/s or step/s required to achieve them, specify the responsible person/s, and the expected outcome/s.
    • Specify the date set for the completion of each action/s or step/s.

 

  1. Consider the following when developing the case management plan:
    • All available services are explored and the most appropriate ones are included in the
    • Ensure that the responsible person/s’ and/or service/s’ experience and capability matches the support needed by the
    • Ensure that the service providers you have identified are accessible to the client and to you, as the client’s case
    • Use an inclusive approach when developing the

 

4. Facilitate a Case Conference

This part of the assessment will require you to develop the Agenda and prepare for the case conference. You are also required to facilitate the case conference and ensure that it remains focused. Arrange with your supervisor access to at least two (2) co-workers/ colleagues/ other case managers who will attend the case conference. Your supervisor must also be in attendance for him/her to observe your case conference facilitation.

 

Steps to take:

  1. Prepare for the case conference by:
    1. Securing the attendance of at least two (2) co- workers/colleagues/other case managers who will attend the case conference
    2. Reserving a room in the service for the venue of the conference
    3. Reproducing the developed case management plans (ensure that the number of copies is enough for the attendees)
    4. Creating the meeting agenda, including the following details:
      • Description of the meeting
      • Date and time start/finish
      • Venue
      • Attendees
      • Agenda item, who is assigned to discuss it, how long should it be tackled, and time start/finish for each

 

  1. Facilitate the case conference. During the preliminary part of the meeting, you must be able to:
    1. Initiate a round of introductions and provide the purpose/aims of the
    2. Clarify the allotted time for the meeting, and state that you will be facilitating and taking the minutes. Ensure all the attendees that a copy of the minutes will be sent to all relevant service providers involved in the
    3. Distribute the Agenda and the case management plans to all
    4. Explain how the case conference will go about to all the attendees. (e.g., conference will be divided into two parts as two case management plans will be discussed and reviewed).
  2. Discuss the Agenda and outline the case history of the
  3. Facilitate the meeting. Restate that the main objective of the meeting is to discuss how everyone in attendance will work together, supply support to assist the client, and make an agreement to monitor the client’s progress toward achieving the set

If the discussion focus shifts from the main objective of the meeting, your role as facilitator is to remind the group of the current task in hand and the required outcome.

  1. Cover the following during the meeting:
  • Roles, responsibilities and boundaries of workers/ service providers/ organisations are defined
  • Needs, rights and responsibilities of the client, client’s family (if applicable), community, and service providers are identified and agreed upon
  • Processes of coordinated/integrated service delivery and service provision requirements are explained
  • Case management plan is reviewed with the service providers:
    • Prioritised needs of the client are communicated to all service providers
    • Realistic targets for change or action for client are set
    • Barriers to attaining outcomes are identified
  • Collaborative working    arrangements   and    requirements   are determined:
  • Service duplication must be identified
  • If duplication is identified, steps on how to manage these must be outlined
  • If there are case management plan issues that cannot be met by the services present, it should be identified which services are to be contacted and who will undertake this role
  • Procedure on how to coordinate must be explained to the client to minimise stress and confusion
  1. Ask all the attendees for their feedback on the identified recommendations and the emerging case management
  2. Identify the case meeting outcomes by clarifying the responsibilities and timeframes for the attendees. The process for monitoring and reviewing outcomes must also be established. Ensure that these are recorded and agreed upon by everyone in
  3. Summarise all points discussed in the
  4. Close the meeting by informing all attendees that the minutes and copy of the revised case management plan will be provided to
  5. Document the case conference using the Meeting Minutes template provided on the next pages. Prior to submitting minutes to your assessor, request all the attendees, including your supervisor, to date and sign
  6. Revise the case management plans based on the recommendations gathered from the case conference.

1. Debriefing and Crisis Procedures

For this part of the assessment, you will be required to gather information about the organisation’s policies and procedures relevant to crisis procedures and debriefing process.

 

For this assessment, you will require access to:

  • Policies and procedures of the organisation for at least one (1) crisis situation

Guidance: Crises situations may include but not limited to: potential suicide/suicide ideation of client, domestic violence, self-harm, and behaviours of concern demonstrated by a client.

  • Policies and procedures of the organisation for staff debriefing
  • List of available resources for debriefing, and crisis procedures
  • Legal and ethical considerations relating to debriefing, and crisis procedures

 

Steps to take:

  1. Ensure that you have discussed with your supervisor the resources required for this assessment and have secured necessary permissions to access information from the organisation.

 

  1. Identify one (1) crisis situation to which the Vocational Placement Organisation responds to as part of their

 

  1. Identify two (2) indicators of risk to self and two (2) indicators of risk to others brought about by the crisis

E.g. If a client had attempted suicide, the client may show physical signs such as unexplained injuries.

 

  1. Provide information about the policies and procedures relating to this crisis situation, including the following:
    • How the organisation will respond to the situation
    • Reporting requirements (e.g. details required for the incident)
    • Reporting procedures
    • Person/s responsible for taking action on the situation

 

  1. Provide information on the organisation’s process for debriefing staff who have experienced/witnessed critical incidents or crisis situations. Guidance: Outline the procedures for staff

 

  1. Identify three (3) resources that the organisation has for crisis procedures, and three (3) resources for staff debriefing.

  • Creating an Action Plan with your colleagues to address the current and potential areas of need in the workplace
  • Identifying the professional and personal performance standards of your colleagues and use them to monitor their wellbeing and performance

 

For this assessment, you will require access to:

  • Two (2) colleagues who have experienced work-related stress in line with their roles as case workers or case managers

Guidance: Where possible, they can be members of a team whom you will be working with during your Vocational Placement. This does not have to be major stress issues – it could be as simple as not being able to complete all duties within the working hours, etc.

 

This assessment is divided into two (2) tasks:

  • Task 1 – Stress Questionnaire
  • Task 2 – Action Plan and Performance Standards Checklist

 

Prior to commencing this part assessment, ensure that you have discussed and confirmed with your supervisor and two (2) colleagues the participation required of them for this assessment.

Instructions are provided for each part of the task to guide you in completing this assessment.

 

Task 1 – Stress Questionnaire

For this assessment, you are required to have your colleagues complete a Workplace Stress Questionnaire.

 

Steps to take:

  1. Have each of your two (2) colleagues complete a Stress Questionnaire provided in this workbook. Once completed, have them return to you the form.

Guidance: Ensure that you collect the completed forms as soon as possible as you will require their responses for the succeeding task.

 

  1. Ensure to maintain the privacy and confidentiality of the information provided by your colleagues, however, the questionnaires must be signed off by your supervisor to ensure validity and authenticity of the participants.

 

Task 2 – Action Plan and Performance Standards Checklist

For this assessment, you are required to:

  • Create an action plan with your colleagues to address the current and potential areas of need in the workplace
  • Discuss with your colleagues about their professional and personal performance standards in line with their work role

 

Steps to take:

  1. Schedule a discussion with your two (2) colleagues who responded to the Ensure that you have secured necessary permissions from your supervisor and colleagues prior to commencing this task.

 

  1. In your discussion, clarify with your colleagues your work role and the purpose of the meeting. Inform them that the data gathered relating to them is for assessment purposes only. Ensure them that you will maintain the privacy and confidentiality of their

Note: When developing the team performance plan, ensure to inform your supervisor and your colleagues that the plan you will develop with them will not necessarily be used by the work team when performing their tasks in the organisation, but only as part of this assessment. However, you may seek their permission to use their current team performance plan as basis for completing this assessment.

 

  1. Complete the Team Performance Plan template provided in this Your plan must include the following:
    1. Name of team members
    2. Plan period, i.e. the timeframe set for completing all the tasks in the plan
    3. Two (2) key performance indicators for the team
    4. Two (2) goals of the team relevant to the organisation
    5. One (1) strategy each to achieve each goal specified
    6. One (1) expected outcome when the strategy has been implemented
    7. Person/s responsible for implementing the strategy
    8. Expected timeframe for completing the task

This assessment will require you to undertake Consumer Participation and Engagement. This assessment is divided into three (3) tasks.

Task 1 – Consumer Participation Audit

 

Steps to take:

  1. Access and review your organisation’s policies and procedures on consumer

These may have already been provided to you on your first day of Vocational Placement or during your induction and orientation in the organisation.

 

  1. Fill out the Consumer participation audit template. Conduct the audit by interviewing two (2) consumers of the organisation: 1 former and 1 current or 2 current consumers.

 

Supplementary Questions

1. What philosophies or attitudes does your Vocational Placement Organisation have in place for consumer participation? Provide at least two (2) philosophies or attitudes.

2. Based on your responses in Question 1, select one (1) philosophy and briefly discuss if it is written or outlined as a policy in the organisation. Provide only a summary.

3. As a case worker, how do you think consumer participation affects the overall delivery of service in the organisation?

 

Task 2 – Discussion of Audit Outcomes in a Meeting

This part requires you to conduct a meeting with at least two (2) staff members to discuss the outcome of the audit that was previously conducted.

 

Steps to take:

  1. After conducting the Consumer participation audit, gather at least two (2) staff members to discuss the outcomes of the audit that the candidate has

 

  1. Consider the following questions in the discussion:
    • What areas are important to focus on in developing consumer participation in your organisation?
    • How will outcomes of the audit be shared to staff and consumers that were not able to participate?
    • Are there opportunities for them to comment on the priorities?

Guidance: Note that it is important that these questions be answered because the needs and priorities that we want to be raised regarding consumer participation should be representative of the staff’s and consumers’ thoughts and not just by a selected few.

 

  1. Identify the priorities that can be worked on and can be addressed Based on the levels of participation in the Audit form, priorities can be identified.

 

Task 3 – Consumer Engagement Plan

This part requires you to create a Consumer engagement plan based on the priorities that were identified in the Consumer participation audit previously conducted.

The consumer engagement plan will describe the activity that will be implemented to ensure that the consumers are able to participate and be involved in the organisation.

 

Specifically this will cover:

  • Time frame, purpose, goals and objectives of the activities
  • Target audience and type of activity
  • Outcomes and measures of success
  • Resources and communication strategies
  • Evaluation of the plan

 

Steps to take:

  1. Locate the Consumer Engagement Plan provided in the next
  1. Create the   plan    based    on   the   previously   conducted   consumer participation audit.
  1. Have your supervisor sign the completed Consumer Engagement Plan.

 

Supplementary Question

1. Based on the audit and the meeting you performed, explain why you have chosen the specific consumer need to be the focus of your Consumer Engagement Plan. Explain in not more than 150 words.
